                 | To become a new patient, call ACCESS Family Care to schedule your first appointment. You will be asked to provide some basic personal and insurance information. We will also ask for your email address and cell phone number for appointment

                    Complete Patient Registration Forms

                            Print out and complete the Patient Registration packet and Patient Portal Consent below.  Bring your completed forms to your first appointment.  During your first appointment you will be
                        
                        
                            given a one-time Personal identification Number (PIN) that will allow you to Register on the Patient Portal.
