ACCESS Family Care has served our communities with primary care practitioners and dentists since 1996. We collaborate for hospital admissions with area hospitals and we maintain a referral network of specialists. Do we take your insurance?

We are network providers with Aetna, Blue Cross Blue Shield, CIGNA, Humana, United Healthcare, MO HealthNet (Medicaid), Medicare and other insurance plans.

To become a new patient, call ACCESS Family Care to schedule your first appointment. You will be asked to provide some basic personal and insurance information. We will also ask for your email address and cell phone number for appointment reminders.

Complete Patient Registration Forms


Print out and complete the Patient Registration packet and Patient Portal Consent below.  Bring your completed forms to your first appointment.  During your first appointment you will be given a one-time Personal identification Number (PIN) that will allow you to Register on the Patient Portal.
